4/10/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival to the home setting, Ms. Hodge discussed with QMHP-C Ashiyah has been struggling with peer interactions with her siblings and her peers. Mother reported since the last session, Ashiyah has been in verbal altercations with her siblings which has also contributed to physical altercations within the home. 2. Behavior: Ashiyah appeared to be receptive to the interventions provided to her. Ashiyah expressed to QMHP-C during the session, she only engages in behaviors at which people show her. She continued to state during the session, her siblings get on her nerves and when she is in the school setting or when she is out in the community people don’t know how to talk to her; therefore, she is going to say what she has to say. 1. Intervention: QMHP-C openly discussed positive peer interactions and triggers that causes feuds with siblings. QMHP-C processed with Ashiyah on periods of time where she managed behaviors to avoid conflicts. QMHP-C reviewed calming skills with Ashiyah before acting out negative with unacceptable behaviors. 2. Intervention: QMHP-C modeled conflict resolutions and problem-solving behaviors and role-played showing different techniques to demonstrate appropriate ways to show feelings and express thoughts. QMHP-C reminded Ashiyah on how her behaviors impacts her mother, and siblings. QMHP-C and Ashiyah talked about her negative behaviors and self-control. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah expressed that she will fight and become argumentative with siblings, and her mother. Ashiyah reported that she siblings try to control her and fusing with her which trigger her defiant behaviors in the home. 2. Response: Ashiyah reported that her sisters make her cry when they yell and scream at her. Ashiyah was able to demonstrate positive behaviors during role-play and verbalizes an understanding of how her behaviors are projected onto others.
